A Huge Recurrent Pheochromocytoma Demonstrated by I-131 MIBG Scintigraphy

Abstract
A 56 year-old man was referred to the department of nuclear medicine for the evaluation of a 7.5 cm sized lobular soft tissue mass on abdominal CT (Fig. 1). His chief complaints were facial flushing and chest discomfort for about 4 months. He had a history of left adrenectomy and nephrectomy for pheochromocytoma 8 years ago. On the physical examination, a 7 x 5cm sized nontender mass was palpable in his LUQ of
the abdomen.
1-131 MIBG scintigraphy (Fig. 2) showed a huge uptake in the left renal area with multiple scattered hot spots in the left abdomen. Tc-99m MDP bone scan (Fig. 3) showed multiple hot spots involving right 8th rib, left 4th, 6th, 8th ribs, L3, and right ischium. Left kidney was not visualized on the bone scan due to prior nephrectomy.
